Reissue Certificate

In order to reissue a certificate for a course you have completed, we will need the following information emailed to us.

Name as it should appear on certificate:

Date of course:

Name of course:

Location of course:

Order number:

After you have paid the administration fee, please copy and paste the above, complete and email to info@cuppingcanada.com

Please allow 1-2 business days to receive your certificate by email.
